Required Application Support Documents to be submitted with application:

1. Income verification documents such as pay stubs, vouchers, benefits details, etc.
2. Clear picture of a photo ID for each adult applicant.
3. Application fee for each adult applicant.
4. Any other relevant and discussed supporting documentation.
5. Completed and Signed Screening Criteria Page
6. Completed Screening Questionnaire
